We collect personal information from you, including:

• your name
• your contact details
• your interactions with us (the wording, dates and times of the messages you send to the clinic when using the app)
• dates of your appointments and your feedback on each appointment if you choose to provide it

We collect this information to remind you of your appointments and improve our services.

Providing some information is optional but if you choose not to enter appointments, we'll be unable to provide reminders.

If you choose not to enter feedback on your appointments we'll be unable to use this information to improve our services.

We keep your information safe by only storing anonymised data on our servers and only allowing a small number of  specifically approved staff to access this information. We will store the data for a minimum of 10 years, and up until 10 years after you turn 16.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. If you’d like to ask for a copy of your information, or to have it corrected, please send an email.
