What is the rheumatic fever care plan?

The rheumatic fever and rheumatic heart disease information booklet by the Heart Foundation has a care plan in it. It has a checklist to complete before your child leaves the hospital. 

Before leaving the hospital, make sure you've had all your questions answered. You can expect to:

  • understand what has happened to your child's heart
  • know about local support available to help your whānau make the home warmer and drier
  • have talked to a social worker or a Māori, Pacific health and cultural support worker about what support is available for you and your whānau
  • know what to do to get your child's next penicillin injection
